I work for an animal rescue centre and we are trying to come up with a new form for people wanting to re-home their dog/cat. What questions should we ask the people/what information should we ask them to provide? We have the basics like breed, age, sex, good with kids, cats, dogs and livestock, is the dog micro-chipped, de-sexed, vaccinated and registered. Can anyone think of anything else that should be included?What information should you provide when trying to re-home a dog?What are you currently feeding them?
Are they current on vaccinations? Which ones?
What is your veterinarians phone?
Is the dog current on heartworm preventative?
Where did you purchase the hw preventative?
Does the dog know any tricks?
How does the dog walk on a leash?
Is the dog a fence climber/digger?
What negative and positive traits does your dog have? (Ex: barking a lot)
Does the dog have seperation anxiety?
Is the dog house trained?
Is the dog crate trained?
From THEIR experience can the dog be trusted out of the crate?What information should you provide when trying to re-home a dog?
All that info is good but you are missing the the obvious why they are giving the pet up. Tell them, no judgment, it's just important to know. If he bit someone then that needs to be dealt with. If he pees in the house, he will probably be returned for the same problem.
Also, is he friendly with kids, elderly, good with cats %26amp; dogs or should he be an "only child". Color(s), size, good attributes such as good leash walker.

How long they owned it
Any known health issues
Any behavioral issues or personality quirks
Lifestyle: for dogs, how much exercise are they used to getting
Has the animal been kept indoors, or outdoors
